Thank you for inquiring about Burch Physical Therapy. We look forward to participating in your rehabilitation. Rehabilitation is an essential part of your recovery. Our goal is to assist you in regaining your highest level of function and recovery possible. This requires effort and cooperation from both the therapist and the patient. It is very important for you to be consistent with your appointments to physical therapy. If you have any questions concerning your rehabilitation, please do not hesitate to contact us.

Please bring the following to your initial appointment:

1. Referral/prescription from your doctor, unless faxed to our office by MD.
2. All insurance information (insurance card, work comp information, etc.)
3. Photo Id.
4. Completed paperwork (Please download thr file below for our new patient forms.)

If paperwork is already completed arrive 10 minutes early.
If paperwork has not been completed arrive 30 minutes early.

** Our office takes pride in seeing our patients at their scheduled appointment time. If you fail to bring your completed paperwork to your appointment, we may need to reschedule you in order to honor the appointment times of other patients.
